I was treated with dignity and respect by the staff. The rehabilitation therapy was wonderful got me to the phase of transition to outside therapy. Facility is clean and well maintained. Nursing staff amazing especially working with so many patients. I would highly suggest coming here .

Edit: They just lost about 95% of our clothing, including my wife's two favorite sweatshirts, nice, expensive ones that her mother bought her. Went to the laundry room with an employees to look, they were nowhere to be found among the clean clothes, which means one thing: other residents currently have them in their rooms.

The INSANE lack of planning, prep, strategy, or even basic THOUGHT that goes into so many things here is baffling. I expect we'll never see the clothes again at this point. I'll be here every day, documenting.

Will leave your relatives sitting in pee for hours as they continuously beg to be changed. Pretty harsh. The move is to walk in the patient's room, hear the request for a change, say "Let me go get some help", then disappear for a few hours. This will repeat ad nauseum, all night. My wife has been sitting in pee for about 4 hours, we're still trying to get someone to help change her.

Our group loved the ability to use your beautifully decorated conference room to celebrate with our friend who resides at your facility. Not only did you allow us to use your space but you provided the beverages for our celebration. Thank you! I have visited on several occasions and the staff is always friendly and the location is spotless. What a beautiful place to get the necessary care.

We have had our loved one in rehab for two months. In this time, I have formed professional relationships with admin, nursing staff, physical therapy and on call doctors. Without exception everyone has the best interest and physical welfare of the patient at the forefront of their mission statement. The facility itself is clean, beautiful and very well maintained. Our loved one is 97 years old and he was valued and treated with the utmost respect. For that, I will be forever grateful.

Would not recommend going to this facility. It looks like a nice place but the care is lacking. My grandfather moved to the facility in 2019 and it was lovely. He received excellent care as he transitioned to hospice and passed. When my grandmother needed a skilled nursing facility to go to after a hospitalization, we figured it made perfect sense to go back. We were wrong. The physical therapists seemed good, though not consistent with the therapy schedules. Some of the CNA’s were excellent and a couple were frighteningly bad. The RNs across the board were unpleasant and inattentive. I assume they are understaffed and this may be the cause. But, even when I attempted to contact the director (who had left me his cell phone number after a particularly concerning night where no care was as provided overnight and my grandmother was left for hours bed bound with no one checking on her despite using her call light) as well as the nursing supervisor, I got no response. When my grandmother‘s condition suddenly changed, we were unable to get a physician to see her, were advised by the nurse practitioner that consultation with her infectious disease doctor was not necessary, and that nurse practitioners can do all the same things that doctors can do. They refused to come see her as her condition worsened because they’d already left for the day. We asked several times for the on-call doctor to be notified and were told that sometimes they don’t call back, which is unacceptable. It is still unclear to me if the on-call doctor was never contacted by the staff at Stonegate or simply did not call back. I suppose the blessing was that she was ultimately readmitted to the hospital and didn’t die in that awful place.

Unfortunately, my grandmother was brought to this facility. The food they provide is poor quality and cheap for sure, and definitely bland. Makes ya wonder how much and what is actually billed to insurance. Patients aren’t getting good food nor the nourishment that should be getting for the money. I would never visit or use this facility again. I definitely advise to stay away if at all possible.

Do not send your loved one here! My mother entered this facility 10 days ago from the hospital for rehab. We have STRUGGLED to get her the care she deserves. We have spoken to the head of the facility, and the head of nursing and they have done better BUT what happens if a family is not CONSTANTLY advocating for their loved one! God help the person who enters this faculty and has NO one advocating for them! It is 6:45 pm and my 95 year old mother has been ringing her buzzer for help for over an hour. She was to have been served dinner 1 hour and 45 minutes ago! I finally called the facility to seek help. When they entered her room they told her they haven’t come to her aid (despite her repeatedly ringing her buzzer) because her buzzer isn’t working. What would have happened if this had been an EMERGENCY ?
I will add that my mother LOVES her rehab therapist. Also many of the nurses have been extremely kind to her. She has also enjoyed the food they serve. It’s Monday night, strangely we had some problems with her care last Monday night as well. Possibly it’s a different staff on Mondays.

I have nothing but amazing things to say about this facility, from the moment we got greeted by the amazing Sheila the concierge, untill the moment we got discharged by another amazing member of the team by the name of Jason, everything has being outstanding! Heidi the nurse manager always made sure that everything was taken care of with her forever smile. Kenny the night nurse and Jocelyn were a pleasure to deal with. Carolyn our N.P. was always willing to listen and help. I truly think the reason why my mother got well so quickly, is because of the caring and outstanding staff they have, as far as physical therapists and occupational therapists like Sara, Michelle and Masako to name a few, who really, really care about their patients. Hats off to the many CNAs, too many to mention, which I rather not do so I don't miss anyone. Kudos to the kitchen staff as well, as they were always happy to help out. I would definitely recommend this facility to someone looking for a 5 ⭐ ⭐ ⭐ ⭐ ⭐ facility on the south side of the city.
